FIRST VISIT WITH US

At your first visit we will prepare your medical file and check your insurance benefits, so it may be of interest that you arrive 15 minutes earlier than your appointment time. It would be very helpful to save time if you fill in at home the personal and medical questionaire online and send us via email or bring it printed to your appointment. Please bring your valid identity certificate, such as ID card, passport, driving license, student card etc. If you are also an international medical insurance member, please bring your insurance card or insurance certificates as well.
